Connect And Care LLC is a Minnesota-based human-services organization offering general CFSS information, adult guardianship referrals for review, and Social Security Advocacy Services.
Our purpose
Connect And Care LLC’s mission is to help older adults, people with disabilities, and people with limited English proficiency access respectful, person-centered, and culturally responsive social services that support dignity, independence, choice, and community participation.
Connect And Care LLC is a Minnesota-based human-services organization. We provide clear information and practical next steps while keeping each service path and its boundaries distinct.

Social Security Advocacy Services
Assistance with understanding SSI and SSDI disability benefits, organizing application information, and navigating the application process.
Referrals are reviewed; case acceptance and benefit approval are not guaranteed. Social Security makes benefit decisions.

Adult Guardianship Services
Now accepting adult guardianship referrals for review. This service is limited to adult guardianship of the person, subject to case review and court appointment when required.

General CFSS inquiries are welcome. Official CFSS consultation referrals are not currently being accepted.

A referral is reviewed individually.
A referral does not mean Connect And Care LLC has accepted the case or agreed to serve as guardian. Each referral is reviewed for appropriateness, conflicts, capacity, legal requirements, less-restrictive alternatives, and court appointment when required.
A referral does not guarantee case acceptance or a guardian appointment.

We welcome inquiries and referrals for people facing barriers to accessing Social Security disability benefits, particularly:
- People with disabilities and limited income.
- People experiencing homelessness or at risk of losing housing.
- People at risk of institutionalization.
- People with serious mental-health conditions or co-occurring conditions.
- People with chronic or complex medical conditions.
- People who need language assistance.
- Communities of color, Tribal communities, and other underserved populations.
- People in rural areas or with limited access to medical, legal, or advocacy resources.
People receiving public assistance or other support services, their families, caregivers, and referring providers can contact us to discuss available assistance.
Services and referrals are reviewed individually. Contacting Connect And Care LLC does not mean that every referral will be accepted.
