Connect And Care LLC is a Minnesota human-services organization developing Community First Services and Supports (CFSS) Consultation Services.
CFSS can involve new language, several roles, and meaningful choices. We center participant choice and person-centered planning, using culturally responsive, accessible language to make accurate Minnesota CFSS information easier to understand and use.
Our approach begins by listening. We believe good support does not take over a person’s decision. It creates room for questions, makes tradeoffs visible, and helps the person identify what matters before a plan is written or a model is chosen.
Important: Connect And Care LLC is not Minnesota DHS, a lead agency, or an FMS provider. It is currently developing CFSS Consultation Services and must not be represented as DHS-authorized, contracted, or enrolled unless that status is confirmed.
