Participant rights and choice

Your choices belong in the room.

CFSS is designed around participant direction. Understanding your rights can help you ask questions and take part in decisions about support.

People using CFSS have the right to:

  • Choose or change service options, with applicable program limits and processes.
  • Choose providers and participate in choosing workers.
  • Participate in person-centered planning and developing the service delivery plan.
  • Receive information about options, responsibilities, costs, staffing, and how to raise concerns.
  • Ask for communication in a preferred method and request accessible or translated information.
Choice includes questions

You can ask what a provider does and does not do, what responsibilities come with a model, and how to change a provider or service choice.

For the current rights and responsibilities, read Minnesota DHS materials directly.

Bring your questions, not private records.

A general question is enough to begin. Please do not send medical, financial, Medicaid, or participant-record information.
