Connect And Care LLC is developing CFSS Consultation Services for Minnesota participants and families. This page describes the support a consultation services provider may offer; it does not mean Connect And Care LLC is currently authorized, contracted, enrolled, or available as a DHS-approved provider.
The role in one sentence
The participant remains in the lead.
Participants direct their CFSS services and write their own CFSS service delivery plans. If a participant wants consultation support, a consultation services provider may offer education, development assistance, plan review, ongoing support, and any required coordination or submission only to the extent the participant wants and according to Minnesota CFSS requirements.
Connect And Care LLC is not Minnesota DHS, a lead agency, or an FMS provider. It does not determine eligibility, authorize services, manage participant funds, or replace the official responsibilities of Minnesota DHS, the lead agency, an FMS provider, or a managed care organization.
